Finding the right hospice care is complex. For too long, families and hospital staff have had to guess at service areas using outdated ZIP code lists or broad county lines. They struggle to find a hospice equipped to manage continuous IV medications for heart failure at home, or one that supports a patient's choice for Medical Aid in Dying (MAiD). This wastes valuable time, creates frustration, and adds to an already overwhelming burden.

PalliAtlas™ is a free, map-based directory built to fix both problems. Our simple, visual map shows exactly where each hospice provides care. In addition, our platform allows hospices to share—publicly, or confidentially with verified clinicians—the specific, advanced services they offer. This allows case managers, hospitals, and families to stop guessing, see real coverage areas, filter by specific needs, and find the right fit, faster.

Built by Hospice Physicians

PalliAtlas wasn't built by a marketing company. It was designed from the ground up by hospice physicians who live the referral problem every day. We are clinicians first. We built this tool for our colleagues and our patients. Our goal is simple: make the right hospice care easier to find and understand for everyone involved.

How It Works

For Patients & Case Managers:

The PalliAtlas map is 100% free to search. You can visually see which agencies serve a specific area or address, and even filter by what services they offer. This eliminates a lot of the guesswork involved in matching the right care to the right patient.

For Hospice Agencies:

Your profile is already on the site. We have created a basic profile for every licensed hospice in our active states (Arizona, Colorado, and New Mexico).

We strongly encourage you to register for a free account to:

  • Confirm and edit your service area. Move beyond basic ZIP codes and show the actual polygons your agency covers. You can modify these as your hospice grows and changes over time.
  • Highlight key programs. Add specialties (like pediatrics or concurrent care for veterans) and list the advanced or unique therapies you provide.
  • Update your profile. Add correct contact details, website links, and photos to your listing.
  • Connect with referrers. Ensure that referral teams and families see accurate, up-to-date information about your agency.

Our Vision for Clinicians (Coming Soon)

We are currently developing enhanced features specifically for our hospital and clinic colleagues. Soon, verified case managers, social workers, and other clinicians will be able to create their own secure PalliAtlas accounts.

These professional accounts will provide access to a deeper layer of information that hospice agencies can share privately with referrers, including:

  • Detailed Clinical Capabilities: Specifics on complex wound care, IV infusion services, or other specialized clinical programs.
  • Key Areas of Expertise: Find agencies with documented expertise in managing medical aid in dying (MAiD), complex cardiac patients, or specific palliative treatments.
  • Verified Intake Contacts: One-click access to the correct, verified intake phone numbers, so your team can send referrals quickly from your own system.
  • Meet Compliance Rules Instantly: Fulfill Medicare's "freedom of choice" requirement. Our platform will generate a printable list of all agencies that serve your patient's address and service requests, ready for the patient to choose from.
